Plan Your Visit

Whether you have an appointment or are coming in for immediate care, here are some tips to help you ensure you get the care you need.

What To Bring

  • Photo identification: This can include a driver’s license, state-issued ID, passport, work- or school-issued ID or CASA ID.
  • Proof of insurance: We accept most insurance plans (see list below), as well as Medicare and Medicaid.
  • If you don’t have insurance:  We can help! If you don’t have health insurance, we’ll help you find resources, and assist with insurance and/or provide access to care at a discounted rate. Just complete a Sliding Fee Scale application and bring proof of income. Click here for details and to download the application.

  • Others may be accepted upon verification
  • Dental: Most Maryland dental plans are accepted

Payment for Services

Co-pays are due at the time of service. However, as a community health center, we provide services regardless of ability to pay. We offer a sliding fee scale (discounted services, see below), as well as support services to help you get financial assistance, insurance coverage, prescription assistance and more. Contact us online or at 410-383-8300 prior to your appointment to learn more.

Applying for Sliding Fee Scale (Discounted Payment Program)

If you need assistance in paying for any Total Health Care services, you may be eligible for our discount payment program. You will need to bring in documents to establish eligibility for the sliding fee scale.

To establish a sliding fee scale at Total Health Care you will need:

1. Family Size:

  • List of names and birthdates for all household members or anyone supported by the household

2. Proof of Income: Bring in all that apply from the following list:

  • W-4 and / or a copy of current year’s Income Tax Return
  • Copies of two pay stubs for bi-weekly pay or four pay stubs or weekly pay
  • Notarized letter from employer stating total amount of the last four paychecks or expected earnings for the next six months
  • Unemployment award letter
  • Current year pension benefits letter
  •  Current year SSA/SSI/DSS award letter
  •  Current year AFDC award letter
  • PAC denial letter

3.  Proof of NO Income: Bring in ONE of the following: 

  • “Verification of No Income” form (available at front desk)
  • Letter stating patient has no income signed by both the patient and a valid reference

If you would like to apply for benefits, or check your eligibility for benefits, you will need to bring your brith certificate, your social security card and your state identification.

Division Health Center Closing

August 9, 2024

Total Health Care’s Division Health Center will be closed at 12 noon. All patients will be contacted to be seen at another site or appointment rescheduled.
