As a Total Health Care, you’ll experience a special kind of care. It’s called the Patient-Centered Medical Home (PCMH) approach to care. It’s a nationally recognized model that’s designed to deliver a better health care experience and better health outcomes for you and your family.
How It Works
It starts with connecting you to a primary care provider. He or she will direct a team of Total Health Care specialists who are all working as one team for you.
Together, your team will write a custom care plan for you to help you achieve your best health.
Your team will provide continuous, coordinated care throughout your lifetime to ensure you get the best care possible, no matter what you need.
Your team will also coordinate all the services you need, when you need them—from doctors’ appointments and tests to medication reviews and social services.
Because it’s “patient-centered,” you are always in charge of your care. Your health care team is there to educate you and help you make the best decisions for you and your family.
