Total Health Care nationally recognized model of care

Patient-Centered Medical Home

As a Total Health Care, you’ll experience a special kind of care. It’s called the Patient-Centered Medical Home (PCMH) approach to care. It’s a nationally recognized model that’s designed to deliver a better health care experience and better health outcomes for you and your family.

How It Works

  • It starts with connecting you to a primary care provider. He or she will direct a team of Total Health Care specialists who are all working as one team for you.
  • Together, your team will write a custom care plan for you to help you achieve your best health.
  • Your team will provide continuous, coordinated care throughout your lifetime to ensure you get the best care possible, no matter what you need.
  • Your team will also coordinate all the services you need, when you need them—from doctors’ appointments and tests to medication reviews and social services.
  • Because it’s “patient-centered,” you are always in charge of your care. Your health care team is there to educate you and help you make the best decisions for you and your family.
