With Total Health Care’s primary care team, you have a partner dedicated to helping you manage all your health and wellness needs, at every stage of your life. Our board-certified primary care doctors and family practitioners are specially trained in all aspects of adult health. We will work with you to develop and coordinate a complete plan for your health that’s just right for you.

Services

  • Preventive care, including annual physical exams, immunizations and screenings
  • Acute care to diagnose and treat you when you’re sick
  • Same-day appointments for current patients to help you feel better quickly
  • Late hours and on-call services to ensure you can get the care you need, when you need it
  • Chronic disease management for health conditions such as diabetes, asthma, high cholesterol, high blood pressure, Hep C and HIV
  • Coordinated care with our on-site specialty care providers, including dental care, OB/GYN care, behavioral health care, infectious disease care and more
  • Referrals to the additional medical specialists you need, from cardiologists to orthopedists
  • On-site lab work for blood draws and other screenings
  • Prescriptions for antibiotics and daily medications, which can be filled at our on-site pharmacies
  • Education and guidance on topics like nutrition and medication management to help you take control of your health and make the decisions that are right for you

Division Health Center Closing

Total Health Care’s Division Health Center will be closed at 12 noon. All patients will be contacted to be seen at another site or appointment rescheduled.
