Consent & Registration

A Child Immunization Consent Form needs to be filled out to serve as a record of your consent to receive vaccinations during your appointment. Consent forms can be submitted by uploading the completed form below or bringing the completed form with you to your appointment.

All information collected on the form is strictly confidential and will become a part of your medical record.

FAQs

Vaccines are super important for teens for a couple of big reasons:

  • Serious Illness Protection: Teens are at an age where they’re becoming more social and independent. This can expose them to germs that cause diseases like meningitis, HPV, and hepatitis. Vaccines help teens build immunity (protection) against these diseases, which can be very serious and even deadly.

  • Long-Term Health Benefits: Vaccines don’t just protect teens from getting sick right now. They also provide long-term health benefits. For example, the HPV vaccine can help prevent certain types of cancer later in life.

Yes. Parents must be present to accompany all minors. 

Ages for each vaccine may differ. Please call Maryland ImmuNet at 410-767-6606 to verify your child’s age and the vaccine in question.
